15 Significant non-compliance and escalation
15.1 Our definition of significant serious incidents is any incident which has the potential to result in a potential breach of legislation or regulatory standard, or which causes a risk to health or safety. All serious incidents will be reported and escalated as soon as possible, and no later than 24 hours after the incident occurred, or of a Gateshead Council employee becoming aware of it.
15.2 Any incidents identified at an operational level will be formally reported via the Assyst Health and Safety Incident Reporting System and the process followed, which will include investigating the incident to prevent recurrence.
15.3 Non-compliance notices will be issued for all breaches of health and safety for employees and contractors, as detailed within the Health and Safety Compliance Procedure (OP-15).
15.4 In cases of serious incidents (as defined in 15.1), SMT and Cabinet will consider whether it is necessary to disclose the issue to the Regulator of Social Housing as required by the regulatory framework, or any other relevant organisation such as the Health and Safety Executive (opens new window).
