In Gateshead we are committed to introducing well tested tools and processes with thorough training so practitioners from different agencies are supported to work together more effectively.

We have a holistic model of integrated working linking together the use of the Common Assessment Framework (CAF), the role of Lead Practitioner, the concept of the Team Around the Family, easier and more appropriate information sharing and the roll out of more personalised services for Children, Young People and families through the use of the , the concept of the Team Around the Family, easier and more appropriate information sharing and the roll out of more personalised services for Children, Young People and families through the use of the Personalisation Fund to provide finance for integrated support plans.

Our goal for every child with additional needs is that they would have:

  • A single support plan which details what agencies and the family will do to produce better outcomes
  • A lead practitioner to co-ordinate the support plan and act as a single point of contact helping to reduce duplication
