Registration is important as it gives parents the opportunity to have their baby officially acknowledged and to give their baby a name if they wish to – as well as being an important source of historical and statistical information.
Who can register the still birth
If parents are married at the time of the stillbirth, the mother or father can register the stillbirth on their own.
If parents aren’t married:
- if you want the father's details entered on the register, both parents can go and sign the stillbirth register together;
- if one parent cannot go to the Register Office they must complete a Statutory Declaration form (available from the Register Office) acknowledging parentage from the other partner. The form needs witnessing by a solicitor, who will charge a fee.
If it is difficult for the parents to attend to register the still-birth, other people may be able to attend and provide the information. This might include people in authority from the hospital where the still-birth took place. Please contact us for further information.
Registering a stillbirth
The stillbirth will normally be registered at the Register Office for the district in which the event took place. If this isn’t possible you can go to another office (still within 42 days) and the registrar will send your details to the appropriate district office - who will then send any certificates to you .
You will be given a short birth certificate (baby's first name (s) and surname, sex, date of birth and district of birth) and a form for the funeral director. Both are issued free of charge.
If you wish a full certificate (as short certificate but also including parent details) is available from the Registrar for £4. Charges afterwards vary:

What to bring to the appointment
All you need is the medical certificate of stillbirth issued by the doctor or midwife present at the time (and Form 16 if you need one).You will be asked to provide certain information:
- Date and place of the stillbirth;
- Sex of the baby;
- The first name(s) and surname if the parents wish to name the baby.
- First name(s) and any surname which she has used before and since her marriage (if this applies);
- Date and place of her birth;
- Current occupation;
- If married to the father, date of marriage;
- The number of previous children (by her present and any former husbands)
Father (if information is to be included on the register):
- First name and surname;
- Date and place of his birth;
- Current or last occupation;
Obtaining certificate for an historic stillbirth entry
Due to the sensitive nature of stillbirth registrations, the procedure for ordering a copy of a stillbirth entry differs from other types of certificates. Apart from very recent events, the General Register Office holds records of all stillbirths registered in England and Wales since 1 July 1927. Certified copies of these records can only be obtained with the registrar general's consent.
The following conditions apply:
- A certificate will only be issued to the mother or father of a stillborn child;
- Siblings may apply if their parents are no longer alive. Details of their parents dates of death should be sent with the application;
- Stillbirth certificates can only be obtained from the General Register Office in Southport.
