Memorial Wall
November 2005 saw the launch of a new memorial wall at both Saltwell and Birtley Crematorium. The wall will take over the role of the current system of plaques, which are to be discontinued over a period of time.
The wall plaques are made from green slate and are inscribed in 22 carat gold. There are two sizes of wall plaque on offer and the application forms on the right of the page include examples of three sample tablets.
A ten-year lease is offered on the plaques with a possibility of extending for a further 10 years after this time.

Once you have completed the application form you can either;

Bring it into the Civic Centre where you can pay by cash, cheque or debit / credit card

Post the form along with a cheque or the completed debit / credit card details form (Please do not send cash in the post)
