3. I am in hospital and am concerned about how I will cope when I am discharged is there any help for me?

You need to contact the hospital social worker team, you can do this from any of the wards.  A social care worker will come and see you and undertake an assessment for your needs.  Where you are unable to return home and need rehabilitation to build your independence skills there are options available to help you return to your home. For a few people, returning home from hospital is not possible, if you are unable to return home you may need to go into an assessment bed until you can choose a permanent place to live.
