This team works with children aged 0 - 18 with physical and/or learning disabilities.
The team offer emotional and practical support to children and their families in order to enhance the quality of their lives. This support includes:

how to access adaptations and special equipment;

short break facilities through the Home from Home service and through partner agencies;

support from family centres;

out of school clubs.
The team also works closely with other services for disabled children to ensure families experience a joined up service. A range of services for disabled children from health, education, social care and voluntary sector can be accessed via a single point of access.
These services meet regularly to agree how best they can meet the needs of individual families.
Referral to the team
A referral to the team can be made by anyone, including the parent/carer of a child or young person with a disability, a GP or a Health Visitor. An eligibilty criteria applies to all referrals.
